MONESSEN, Pennsylvania — A fatal industrial accident at the Cleveland-Cliffs coke plant along the Monongahela River in Westmoreland County claimed the life of 53-year-old employee Jeffrey Smith on Sunday afternoon, September 27, 2026.

Westmoreland County emergency dispatchers received a 911 call reporting a workplace emergency at the facility at approximately 4:15 p.m. According to Westmoreland County Coroner John Ackerman, Smith, a resident of Newell in nearby Fayette County, became trapped between two heavy door machines while performing operational duties.
