According to initial reports from investigators, the crash involved an MG ZS passenger vehicle, a Mitsubishi Triton utility vehicle, and a heavy transport truck. The initial impact between two passenger vehicles caused one to strike the commercial truck before colliding with the third vehicle. Two occupants inside the MG ZS sustained fatal injuries and were pronounced deceased at the scene by emergency medical personnel. Air ambulance units airlifted three young occupants of the Mitsubishi Triton—a 17-year-old male learner driver, a 14-year-old female passenger, and an 11-year-old female passenger—to regional medical centers. The 14-year-old girl later succumbed to her injuries in hospital. A 43-year-old female passenger in the Triton and the 29-year-old truck driver escaped without physical injury.

Detectives with the Victoria Police Major Collision Investigation Unit (MCIU) have established an active investigation into the crash.
Investigating officers closed the Western Highway in both directions overnight between Stawell and Ararat to conduct crash scene reconstruction and process physical evidence before reopening the roadway on Monday morning. Police are examining contributing factors, including driver fatigue, speed, and potential distraction. Formal identification of the two deceased MG ZS occupants remains pending completion by coroner officials.
